What is Gynecomastia?
Gynecomastia, or male breast enlargement, is a medical term derived from the Greek Words meaning “women-like breasts.” This medical condition affects an estimated 90% of adolescent males. Although most cases resolve in time, it can persist in a large percentage of men as they transition into adulthood.
For men who feel self-conscious about their appearance, Dr. Hoefflin performs gynecomastia or male breast surgery. The procedure permanently removes excess fat and or glandular tissue from the breasts, and in extreme cases, removes excess skin. The final result is a more flat, firm, and contoured male appearance.
What are the causes of gynecomastia?
Gynecomastia, or excess development of breast tissue, can occur in as many as 40 – 60% of men. In some patients, medications such as digitalis, anabolic steroids, Tagamet, and spironolactone can be the predisposing factor. In others, marijuana, alcohol, or obesity can play a pivotal role. If you are on prescription medicines that might be causative factor, discuss with your prescribing doctor whether there are alternative drugs for you to take.
Who is the ideal candidate for a gynecomastia?
The best candidates for gynecomastia surgery in Los Angeles are those in relatively good shape, healthy, non-smokers, and without significant medical problems, including diabetes.
Is there a minimum age requirement for gynecomastia?
While there is no absolute age requirement, many surgeons prefer to wait until the patient has completed the puberty process.
How is Gynecomastia surgery performed?
Depending upon the degree of breast enlargement, there are several options for removing the tissue. These options may include liposuction of the chest wall, direct excision using a small elliptical incision on the lower half of the areola, or a combination of both. If the tissue is more fatty in nature, liposuction alone can provide excellent results.
Will there be any scarring?
To remove the glandular tissue, a small and well-hidden incision is placed within the pigmented border of the areola and normal chest wall skin (peri-areolar incision). Additional one or two small incisions may be placed to facilitate liposuction to sculpt the chest to the best aesthetic shape and contour. Within 6-12 months, these incisions become nearly invisible, and cannot be distinguished from your normal surrounding skin.
It is possible that gynecomastia surgery may be performed more than once on a patient?
Gynecomastia is occasionally performed more than once on a patient. This is usually the case when liposuction alone was performed, while leaving some glandular tissue behind. In addition, men with very glandular types of gynecomastia, particularly ones related to medications, marijuana, or steroid use, may return if substance abuse persists.
Are there different liposuction techniques for male breast reduction?
There are several liposuction techniques, including standard suction lipectomy, laser-assisted, Vaser Liposelection® (ultrasound), and Power-assisted liposuction. Each technique has it’s own inherent advantages. In all techniques, a small incision is made which facilitates a small suction cannula being inserted. A back-and-forth motion is performed beneath the skin, excess fat is broken up, and then aspirated out of the body.

What is Vaser-assisted liposuction?
Vaser Liposelection® precisely and efficiently removes unwanted body fat using state-of-the-art ultrasound technology. The fat is first liquefied and then gently removed with liposuction. This particular technique offers less trauma, bruising, discomfort, and swelling relative to other liposuction techniques. This technique is particularly effective in more fibrous fatty areas such as the thighs, hips, back, love handles, and chest.
What should I expect during my initial Gynecomastia consultation?
Your initial consultation with Dr. Hoefflin is an opportunity for you to discuss your desired aesthetic goals. It is helpful to bring a list of questions and concerns that can be specifically addressed. During your consultation, customized literature that discusses the procedure will be provided. Additionally, digital photographs of similar patients will be reviewed. Dr. Hoefflin believes digital photography can be a valuable tool in understanding the benefits and limitations of gynecomastia surgery.
How is a gynecomastia or male breast surgery performed?
Dr. Hoefflin performs gynecomastia surgery on an outpatient basis. Incisions are typically well concealed within the pigmented border of the areola and normal chest skin (peri-areolar). During the procedure, excessive fibrous fatty tissue is removed. Depending upon the extent of the procedure, drains may be placed. The procedure usually takes one to two hours, depending upon the level of sculpting required. All operations are performed in fully credentialed and state-of-the-art surgical facilities
What types of anesthesia are used?
Dr. Hoefflin performs his Los Angeles gynecomastia surgery as an outpatient procedure under local anesthesia with deep sedation, as administered by a board-certified anesthesiologist. When small pockets of fibrous fatty tissue are targeted, the procedure can be performed in the office. The procedure usually takes about one hour. All operations are performed in fully credentialed and state-of-the-art surgical facilities.
Does Dr. Hoefflin use drains after gynecomastia surgery?
During your procedure, a portion of your soft tissue will be removed. This will potentially allow for an open space that can accumulate any fluid. Placement of drains can help reduce swelling and promote better wound healing. Dr. Hoefflin typically removes drains several days after the procedure.
What can I expect from the recovery process?
Patients are directly discharged home with compression garments and gauze dressings covering the treated areas. Garment use is encouraged for the first four to six weeks to reduce any post-operative swelling. Patients may experience mild bruising, swelling, and local discomfort that lasts from several days to several weeks after their procedure. Initial discomfort is easily controlled with oral medication.
Should I expect pain after the procedure?
Our patients are surprisingly free of pain due to the long-acting local anesthetics and nerve blocks that are used. Patients describe the recovery process as a mild to moderate discomfort, of which is easily controlled on oral medication. Most patients tell us they feel much better than expected following their surgery and are able to return to work and social activities within several days following surgery.
Do I need to wear pressure garments after surgery?
After your procedure, you will be discharged home with compression garments covering those treated areas. These specialized garments assist in reducing post-operative swelling and maintaining your comfort. Dr. Hoefflin recommends use of these garments for 2-4 weeks after your procedure.
When can I resume working out after my gynecomastic surgery?
Patients can return back to light or non-strenuous workouts after several weeks, with a full return to all activities after four to six weeks. Premature participation in strenuous activities can prolong your swelling and lead to unanticipated complications.
When can I return to work?
Patients can return to work and other regular activities as soon as they feel comfortable, which is usually after several days. If your occupation requires physical labor, then a recuperation period of 10-14 days is recommended.
What can I do to best prepare for my upcoming Gynecomastia surgery?
Dr. Hoefflin and his staff will supply you with a list of pre-operative instructions, including medications to avoid, in the days and weeks leading up to your Los Angeles gynecomastia. Importantly, These instructions include quitting smoking as it can have deleterious effects on wound healing and your final result.
What are the risks associate with gynecomastia surgery?
When performed by a qualified plastic surgeon, the risk of serious problems after a Los Angeles gynecomastia surgery are rare. Most complications are minor, temporary, and are easily resolved with time. Complications may include infection, hematoma, seroma, sensory changes, or the need for additional or revision breast surgery. You can reduce your risk of complications by closely following your surgeon’s instructions both before and after surgery.
Does Insurance cover gynecomastia surgery?
Unfortunately, most insurance companies do not provide reimbursement for gynecomastia procedures.
What can I do to minimize my risk of bruising?
Aspirin, Motrin, ibuprofen, Advil, and virtually all non-steroidal anti-inflammatory agents thin the blood and can promote bruising. In addition, anything herbal, red wine, and homeopathic medications can do the same. To minimize bruising, it’s advisable to avoid these products for 10 days before your procedure.
Why is important to chose a board-certified plastic surgeon?
Your selection of a surgeon is as important as your decision to undergo cosmetic surgery itself. Many patients do not know that in most states, a licensed physician may perform plastic surgery without being board-certified in plastic surgery. This means someone not trained in Plastic Surgery could be performing your surgery.
